Applen Home Care provides assisted living services, including personal care support, medication assistance, meal preparation, housekeeping, and daily living support in a residential home setting. Services are delivered based on individual care assessments and availability.

2. Admission and Eligibility

Admission is subject to a care assessment to ensure we can safely meet the resident’s needs. We reserve the right to accept or decline admission based on care requirements, safety considerations, and capacity.

3. Care Plans

Each resident will have a personalized care plan developed upon admission. Care plans may be updated as needs change, in consultation with the resident and/or authorized family representative.

4. Fees and Payments

All fees must be paid according to the agreed payment schedule. We accept approved payment methods, including CADI, EW, BI waivers, private pay, and long-term care insurance, where applicable. Late or missed payments may affect continued services.

5. Resident Rights and Responsibilities

Residents have the right to be treated with dignity, respect, and privacy. In return, residents and families are expected to cooperate with care guidelines, respect staff, and contribute to maintaining a safe and peaceful home environment.

6. Health and Medical Care

We provide non-medical assisted living support. Residents are responsible for maintaining relationships with their healthcare providers. In emergencies, appropriate medical services will be contacted immediately.

7. Safety and Conduct

For the safety and well-being of all residents, certain behaviors may result in review of continued residency, including actions that threaten the safety, comfort, or rights of others.

8. Limitation of Liability

While we provide attentive and professional care, Applen Home Care is not liable for medical outcomes, accidents, or incidents beyond our reasonable control. Families and residents assume responsibility for disclosed health risks.

9. Privacy and Confidentiality

All personal and medical information is handled with strict confidentiality in accordance with applicable privacy laws and our Privacy Policy.

10. Changes to Terms
